* For your convenience, you can download our new patient forms by clicking the link below. Just print off, fill out and bring to your appointment. If you are not able to do so ahead of time, be sure to arrive 15 minutes prior to your appointment time and you can fill them out in our office.
* If you have recently had any medical treatment or surgery, let us know as it may warrant a telephone call to your doctor, just to make sure there are no contraindications or special concerns (we are looking out for you!). If you are a patient who takes pre-medication prior to your dental appointment (for joint replacement or any other reason), please let us know so that we can make sure you have your medication prior to your visit.
* We will give you a reminder call a few days before your dental visit in addition to sending a text message (if applicable). Just a reminder, at this time, we are not offering text or email appointment cancellations or requests.
